Insurances and Fees

I have contracts the commercial health insurance companies listed below. Do check with your insurance whether or not my services will be covered. I use a billing company Medical Management Support Services to do my insurance billing. If my medical services are not covered by your plan, you would need to pay at the time of service by cash, check or credit card (Visa, MasterCard) or via my web site by PayPal. I will be happy to give you a bill with the medical billing codes to submit to your insurance for a possible out of the network coverage. Whether or not the insurance will reimburse you will be between you and your insurance company, and I will not be able to provide prior authorization, or guarantee of coverage.

At this practice location I am not able to accept Medicare and Medicaid (Oregon Health Plan, Care Oregon).

For the uninsured, initial visit $195, follow up visit $130;

I do offer a sliding scale fee schedule for the uninsured with low income.

Other fees:

Home visits (in areas of Portland close from my office) $225
No-show, or a cancellation less than 24 hours prior to the appointment time $30 first time, $90 subsequently;
Bounced check $30
Filling out forms or writing a letter outside appointment time $35;

Extended telephone consultation and consultation via videoconference (Skype) will be billed per time spent and is not covered by health insurances.

$55 for up to the first 25 minutes
$20 for each additional 10 minute increment with length of a session determined in advance;
